Method:
1. Define the OUTCOMES you want the business to deliver. Write these in the first white row.
2. Determine WHEN these outcomes are required by. Write the dates in the second white row.
3. For each area in the left column, write the degree the item contributes to each of the required outcomes and the timeframes that they will be achieved.
4. If the total of each column (last row) doesn't deliver, as a minimum, the desired outcomes in the first row, then you've got a problem. If you can't fill out the top row, you've got an even bigger problem. So... work out why you're in business and what you want from it in identifiable and measurable terms. Then fill out the rest as a way of assessing the cohesion, congruence and SANITY of your business. This also applies to projects and investments.
